As a patient you will have a regular doctor who manages your ongoing medical care
They will maintain your electronic medical record, including consultation notes, reviewing correspondence from specialists and incoming TEST RESULTS.
Your doctor will remind you when routine checks are due, and recall you if a discussion is needed about abnormal test results.
They will attend to your needs for reports, e.g. insurance medicals, driving licence medicals, pre employment medicals etc.
Shared access to comprehensive computerised records ensures that all our doctors can review essential information about your medical history and manage your on-the-day problems.
The clinic is fully computerised for both administration and medical purposes. We have a secure computer environment protected by a firewall and we perform daily, weekly and monthly backups of our data. Our computerised medical system enables us to provide you with a reminder notice of issues realting to your health e.g.
- Reminders when preventative health assessments are due
- Pap Smears
- Repeat Pathology
- Chronic Disease Review
- Follow up test results
Transfer of Medical Records
If you are a patient at another medical clinic and wish to have your records transferred (which is advisable for your continuum of care) to High Street Medical Clinic, please download this form (Consent for Release of Patient Information), complete the details and bring this form with you to your appointment. Your new GP at High Street Medical Clinic will sign the release form, thus allowing your previous clinic to dispatch your medical history to your new treating GP at High Street Medical Clinic.
From time to time, undergraduate medical students from both Monash and Melbourne Universities spend time at the clinic observing communication and clinic skills in primary care medicine. Students understand the confidentiality requirements of a medical practice. With your prior consent, the student will observe your consultation. No student observations occur without your consent.
High Street Medical Clinic has full accreditation. This means we meet the standards set down by the RACGP and overarching accreditation body (AGPAL) to ensure quality care is provided is a safe and well-organised manner. Accreditation demonstrates the commitment of the practice to continually improve its service and safety for our patients.
