Managing Your Health Long-Term

We provide General Practitioner Chronic Condition Management Plans (GPCCMPs) to help you maintain optimal health and improve your quality of life. Our services support patients with chronic medical conditions through individualised care plans and coordinated treatment.

What is a Chronic Condition?

Chronic conditions are long-term health issues that last for at least six months and require ongoing medical attention and management. Unlike acute conditions, which are severe and sudden in onset but typically short-term, chronic conditions develop gradually and can last indefinitely.

  • Diabetes (Type 1 and Type 2): Conditions that affect how your body processes blood sugar.
  • Heart Disease: Conditions such as coronary artery disease, heart failure and hypertension.

Cost and Coverage

People who are eligible for a Care Plan are provided with up to 4 bulk billed appointments with their GP and 5 separate appointments with our nurses, to help manage their chronic condition.

  • If it is determined that a GPCCMP is right for you, we will work together to develop and implement a comprehensive care plan.
