MyMedicare is designed to make ongoing care more coordinated and accessible.
From 1 July 2025, changes to the Chronic Condition Management (CCM) items mean that if you’re registered with MyMedicare, your new chronic condition care plans and reviews must be done through your registered practice. This supports consistent, team-based care.
For mental health, MyMedicare links you to longer MBS telehealth appointments and easier access to support under the Better Access initiative, which offers Medicare rebates for psychology, social work, occupational therapy, or GP mental health care. Eligible patients can receive up to 10 individual and 10 group sessions per year.
Overall, MyMedicare helps you stay connected with clinicians who know your history best, while still allowing access to other providers for services not tied to registration.

When you register with HSMC, you are nominating us as your primary place of care. This helps support:

Why Some Fees Still Apply
There are new expectations for clinics to bulk bill, but this does not always cover the true cost of providing care.
To maintain high-quality care for our patients, we will continue to charge a fee after 1 November. This allows the GPs and practice team to be here for you long term.
Medicare doesn’t always cover the cost of providing care, so we may charge a fee.
At HSMC, we continue to bulk bill:
- Children under 5
- Patients with a Medicare Aged Pension card
- All other pension and health care card holders receive a reduced fee, typically $30 out of pocket after the rebate.
Some appointments, such as skin checks or procedures, may have different fees. Our reception team can guide you through these anytime.
